Terms & Conditions

CANCELLATION AND NO-SHOW POLICY
Please provide 24 business hours notice if you cannot make your scheduled appointment. Monday appointments must be cancelled by Friday. Any missed appointments or last-minute cancellations will be subjected to the full treatment fee. Exceptions may be considered for unavoidable circumstances.

LATE POLICY
If you you are more than 10 minutes late, we may require you to reschedule and if this is the case, you will be charged the cost of the missed appointment.

NEW PATIENT INTAKE FORM
For new patients, look for the confirmation email with a link to the new patient history form. Fill this out at least 24 hours before your appointment.
